With the right supports and services, we know people with disabilities can thrive in an integrated community setting. We assist individuals transition from institutional settings (nursing homes, intermediate care facilities, incarceration) to a less restrictive living environment that is able to meet their daily needs within their community of choice.

Connection to case management services and funding for Home and Community Based Services
Assistance finding housing
Information and referral for obtaining furniture and household essentials
Coordination of move from a nursing home to an independent home or more independent setting such as a 24hr site home with Home and Community Based Services to provide supports as needed.
